    Summary

    Arteriography may cause delayed cerebral ischemia through sustained platelet activation. This process, indicated by elevated beta thromboglobulin (BTG) levels, suggests a novel pathogenic mechanism beyond catheter emboli.

    Background:

    • Cerebral ischemic events post-arteriography are typically linked to catheter-induced emboli.
    • This study investigates alternative pathogenic mechanisms for delayed cerebral ischemia.

    Observation:

    • Three patients experienced cerebral ischemia 6-48 hours after arteriography.
    • Platelet activation was assessed by measuring beta thromboglobulin (BTG) levels before and 24 hours after arteriography.
    • Two patient groups were studied: Group I (arteriography shortly after venipuncture) and Group II (no arteriography between samples).

    Findings:

    • Seven of eight patients in Group I showed increased BTG levels 24 hours post-arteriography.
    • Only two of eight patients in Group II exhibited elevated BTG levels.
    • Group I demonstrated a significant increase in BTG levels compared to Group II (p < .05).

    Implications:

    • Sustained platelet activation may contribute to delayed cerebral ischemia following arteriography.
    • Elevated BTG levels serve as a marker for this platelet activation.
    • Findings suggest a need to consider non-embolic mechanisms in post-arteriography cerebral ischemia.
