Atrial fibrillation (AF) recurrence after ablation is common. Therapy-resistant AF shows complex conduction patterns and extensive block lines, hindering treatment effectiveness.

- Highly dissociated conduction patterns and significant conduction block characterize therapy-resistant AF.
Implications:
- Current antiarrhythmic treatments may be ineffective if the AF substrate is not fully understood or evaluated.
- Further research into evaluating and targeting the individual patient's AF substrate is warranted.
