Summary

Diagnosing primary central nervous system lymphoma (PCNSL) is challenging due to nonspecific symptoms. F-FDG PET/MRI can differentiate PCNSL from ischemia, improving patient management.

Background:

  • An 81-year-old man with newly diagnosed PCNSL was evaluated using F-FDG PET/MRI.
  • The study aimed to differentiate between PCNSL progression and a concurrent ischemic event.

Findings:

  • F-FDG PET/MRI successfully distinguished between PCNSL lymphoma extension and recent ischemia in the patient.
  • Ischemia emerged as a critical differential diagnosis for apparent lymphoma progression.

Implications:

  • Combined F-FDG PET/MRI enhances diagnostic accuracy for CNS lesions compared to individual modalities.
  • This integrated imaging approach aids in appropriate patient management decisions for complex neurological cases.
