Observation:

  • A 15-year-old male presented with anterior open bite, edge-to-edge transverse discrepancy, and Class III skeletal pattern.
  • Treatment involved premolar extraction, Hyrax expander, and intrusion mechanics with vertical elastics.
  • The patient achieved Class I molar and canine relationships, ideal overjet/overbite, and a straight facial profile.

Findings:

  • The orthodontic mechanics resulted in excellent facial and occlusal outcomes.
  • Posttreatment and 7-year follow-up radiographs confirmed the stability of the results.
  • This case demonstrates successful correction without surgical intervention.

Implications:

  • Well-controlled orthodontic treatment can be a viable alternative to surgery for specific Class III malocclusions.
  • Patient cooperation is crucial for achieving stable, long-term results in complex orthodontic cases.
  • This approach offers a successful nonsurgical option for patients refusing surgical correction.
