Main Methods:

  • A systematic literature search was conducted from 1950 to 2020, including studies with original data from ≥24-hour ambulatory ECG monitoring in healthy adults.
  • Data extraction and synthesis followed Meta-analysis of Observational Studies in Epidemiology (MOOSE) guidelines.
  • Prevalence and standard deviation of common parameters (sinus pauses, conduction abnormalities, ectopy) were analyzed across age groups (18-39, 40-59, 60-79, 80+ years).

Main Results:

  • 33 studies comprising 6466 participants were analyzed.
  • Sinus pauses >3 seconds were rare (<1%) across all ages.
  • Supraventricular ectopy (>1000/24h) increased with age (0% in 18-39 yrs to 6% in 60-79 yrs).
  • Supraventricular tachycardia episodes increased significantly with age (3% in 18-39 yrs to 28% in 60-79 yrs).
  • Ventricular ectopy (>1000/24h) also showed age-related increase (1% in 18-39 yrs to 5% in 60-79 yrs).
  • Non-sustained ventricular tachycardia prevalence ranged from 0% to 2% across age groups.

Conclusions:

  • This meta-analysis provides crucial evidence-based reference ranges for ambulatory ECG parameters.
  • Significant age-dependent variations in ectopy and tachycardia episodes necessitate consideration during clinical interpretation.
  • The findings aim to improve the accuracy and consistency of ambulatory ECG interpretations.
