Epidermal stem cells (EpiSCs) are mainly located at the basal layer of the epidermis. These cells repair minor injuries of the skin and replace dead skin cells. Summary

Dermal onlay grafts (DOGs) effectively treat secondary periorbital hollows (POHs) when other methods fail. This study shows DOGs are a successful rescue procedure for these contour irregularities, with high patient satisfaction.

Background:

  • Primary periorbital hollows (POHs) are age-related lower eyelid/cheek depressions.
  • Secondary POHs are distinct postsurgical contour irregularities with limited treatment options.
  • Dermal onlay grafts (DOGs), using autologous dermis fat, are a surgical option for refractory secondary POHs.

Purpose of the Study:

  • To describe the surgical technique for DOGs in treating secondary POHs.
  • To evaluate the outcomes of DOGs for secondary POHs.

Main Methods:

  • Retrospective analysis of 13 patients undergoing DOGs for secondary POHs over 27 months.
  • Review of surgical technique and patient outcomes.
  • Inclusion of patients with prior unsuccessful filler or fat injections.

Main Results:

  • 13 patients (10 female, 3 male; average age 52) with an average 9-month follow-up.
  • 11 of 13 patients reported satisfaction with hollow effacement, confirmed by surgeon assessment.
  • Minor, self-limiting complications were common; one case of infraorbital dysesthesia resolved with medication.

Conclusions:

  • Dermal onlay grafts (DOGs) show promise as a rescue procedure for secondary periorbital hollows (POHs).
  • The technique yielded good results in this initial cohort.
  • Further quantitative volumetric studies are recommended to validate outcomes.
