Area of Science:

  • Plastic Surgery
  • Otolaryngology
  • Facial Plastic Surgery

Background:

  • Nasal irregularities are a known risk following rhinoplasty, particularly in patients with thin skin.
  • Existing grafting materials like cartilage, temporalis fascia, fascia lata, and AlloDerm are used to address contour deformities.
  • These methods may involve additional surgical sites or dissection, increasing complexity.

Purpose of the Study:

  • To introduce and evaluate the efficacy of a new supracrural ligament graft for primary rhinoplasty.
  • To assess the safety and applicability of this graft in correcting common nasal contour irregularities.
  • To determine if this graft can reduce the need for other grafting materials or procedures.

Main Methods:

  • A supracrural ligament graft was harvested and utilized during primary rhinoplasty procedures.

  • The study involved 49 patients who received the supracrural ligament graft.
  • Graft placement was documented in the nasal tip, radix, and dorsum.
  • Main Results:

    • The average size of the harvested supracrural ligament graft was 0.6 cm × 0.4 cm.
    • The graft was applied to the nasal tip in 49% of cases, the radix in 40%, and the nasal dorsum in 10%.
    • No complications were reported in any of the 49 patients who received the graft.

    Conclusions:

    • The supracrural ligament graft is a safe, simple, and effective option for camouflaging common rhinoplasty irregularities.
    • It is particularly useful for addressing contour issues in the nasal dorsum and tip, especially in thin-skinned patients.
    • Routine harvesting of this graft may eliminate the need for additional materials or surgical sites.
