Summary

Carotid Doppler ultrasonography (CDU) can rarely cause symptomatic retinal artery emboli, leading to vision loss. Prompt management, including anticoagulation, can improve visual outcomes in these rare cases.

Background:

  • Carotid Doppler ultrasonography (CDU) is a common diagnostic tool for evaluating carotid artery stenosis.
  • Retinal artery emboli are typically associated with cardiovascular events or procedures like cardiac catheterization.

Observation:

  • A 60-year-old male experienced sudden vision loss during a CDU procedure.
  • Fundus examination revealed multiple retinal emboli, arteriolar attenuation, and retinal whitening.
  • CT angiography showed severe left internal carotid artery stenosis and left external carotid artery stenosis.

Findings:

  • The patient presented with acute vision loss and ocular pain during CDU.
  • Retinal arteriolar attenuation, whitening, and multiple emboli were observed.
  • Significant stenosis in the left internal and external carotid arteries was identified.

Implications:

  • Symptomatic multiple retinal emboli are a rare but serious complication of CDU.
  • Early recognition and management, including anticoagulation, can lead to visual recovery.
  • Awareness among patients and clinicians is crucial for prompt diagnosis and intervention.
