    Background:

    • Limited data exists on severe acute respiratory syndrome virus 2 (SARS-CoV-2) infection rates among first responders.
    • Understanding SARS-CoV-2 prevalence in this high-exposure population is crucial for public health strategies.

    Purpose of the Study:

    • To determine the seroprevalence of SARS-CoV-2 infections in first responders.
    • To assess the rate of symptomatic versus asymptomatic infections in this occupational group.

    Main Methods:

    • A prospective serial serologic survey was conducted with first responders from emergency medical services and fire departments.
    • Participants provided nasopharyngeal swabs for PCR testing and serum samples for antibody detection at two visits, approximately 3 weeks apart.

    Main Results:

    • Of 296 respondents, 5.4% tested positive for SARS-CoV-2 antibodies; no PCR tests were positive.
    • 71% reported known exposure to SARS-CoV-2.
    • Most SARS-CoV-2 positive individuals were asymptomatic (50%) or mildly symptomatic, with limited healthcare seeking or work absence.

    Conclusions:

    • The study found a relatively low SARS-CoV-2 seropositivity rate among first responders.
    • Asymptomatic and mildly symptomatic infections were common, underscoring the potential for undetected transmission.
    • Further research is needed to evaluate the risk of asymptomatic transmission from first responders to vulnerable populations.
