    Background:

    • Postural stability is a key indicator of balance, crucial for neurorehabilitation.
    • Traditional methods for evaluating postural stability can be complex or costly.

    Purpose of the Study:

    • To evaluate the reliability and validity of a novel virtual reality (VR) system for measuring postural stability.
    • To assess a VR system using an HTC Vive tracker for assessing balance.

    Main Methods:

    • Twenty healthy adults participated, performing balance tasks under four stance conditions (wide/narrow stance, eyes open/closed).
    • Postural sway was measured simultaneously using the VR system and a force platform.
    • Data analyzed included sway path distance, velocity, and confidence ellipse area.

    Main Results:

    • The VR system showed moderate to high reliability (ICC: 0.56–0.90) across conditions.
    • Sway parameters correlated moderately to very highly with force platform Center of Pressure (COP) parameters (r = 0.420–0.959).
    • Bland-Altman analysis indicated good agreement, and the system effectively discriminated between different balance tasks.

    Conclusions:

    • The proposed VR system is a simple, user-friendly tool for evaluating postural stability.
    • The system offers acceptable reliability and validity for balance assessment in clinical and research settings.
