Background:

  • The ABO blood group system is a well-established factor in various bleeding complications.
  • However, its specific association with bleeding after endoscopic procedures remains under-investigated.

Purpose of the Study:

  • To determine if blood group O increases the risk of delayed bleeding following colorectal endoscopic resection.

Main Methods:

  • A retrospective observational study analyzed data from 8,625 patients undergoing colorectal endoscopic resection across four Japanese university hospitals between 2014 and 2017.
  • Delayed bleeding was defined as requiring intervention within 28 days post-procedure.
  • Multivariate logistic regression was used to assess the independent risk of blood group O.

Main Results:

  • Delayed bleeding occurred in 2.96% of patients (255 cases).
  • Patients with blood group O exhibited a significantly higher incidence of delayed bleeding compared to non-O groups (relative risk, 1.62).
  • Blood group O was confirmed as an independent risk factor for delayed bleeding (adjusted odds ratio, 1.60).

Conclusions:

  • Blood group O is associated with an elevated risk of delayed bleeding after colorectal endoscopic resection.
  • Preoperative ABO blood group screening may enhance risk stratification for these patients.
