Summary

Intramedullary headless compression screws (IMHCS) and plate-and-screws offer equivalent stability for proximal phalanx (P1) fractures. This study found no significant difference in displacement between fixation methods in a cadaveric model.

Background:

  • Proximal phalanx (P1) fractures are common hand injuries.
  • Optimal fixation methods are crucial for restoring function.
  • Short oblique P1 fractures present specific fixation challenges.

Purpose of the Study:

  • To compare the biomechanical stability of intramedullary headless compression screw (IMHCS) fixation versus plate-and-screws fixation for short oblique P1 fractures.
  • To evaluate interfragmentary displacement under cyclic loading in a cadaveric model.

Main Methods:

  • Created 30° oblique osteotomies in 24 cadaveric P1s.
  • Fixed 12 fractures with IMHCS and 12 with a 2.0-mm plate and screws.
  • Subjected all specimens to 2,000 cycles of simulated finger motion.
  • Measured maximum interfragmentary displacement using a differential variable reluctance transducer.

Main Results:

  • No statistically significant difference in mean interfragmentary displacement was observed between IMHCS and plate-and-screws fixation throughout the 2,000 cycles.
  • Statistical equivalence in fracture displacement was confirmed at the final cycle.

Conclusions:

  • Intramedullary headless compression screw (IMHCS) fixation provides biomechanical stability equivalent to plate-and-screws for short oblique P1 fractures.
  • IMHCS fixation may be a suitable option for immediate postoperative active range of motion therapy.
