Nursing notes can help detect urinary tract infections (UTIs) in home care. A natural language processing (NLP) algorithm effectively identified UTI signs in these notes, aiding early detection and risk assessment.
Area of Science:
- Medical Informatics
- Clinical Nursing
- Natural Language Processing
Background:
- Urinary tract infections (UTIs) are common in home care settings.
- Standard assessments often fail to capture UTI signs and symptoms effectively.
- Nursing notes contain valuable, yet often overlooked, clinical information.
Purpose of the Study:
- To evaluate the utility of nursing notes in identifying UTI signs and symptoms in home care.
- To develop and assess a natural language processing (NLP) algorithm for automated UTI detection in clinical documentation.
Main Methods:
- A natural language processing (NLP) algorithm was developed to identify UTI-related terms in nursing notes.
- The algorithm utilized the Unified Medical Language System (UMLS) and was trained on expert-annotated clinical notes.
- Performance was evaluated using a gold standard dataset and compared against emergency department (ED) visit or hospitalization data.
Main Results:
- The NLP system demonstrated high performance (F-measure = 0.9) in identifying UTI-related information.
- UTI-related information was significantly more prevalent in home care episodes with subsequent ED admission or hospitalization (81%) compared to the general population (21.6%).
- Documentation of UTI-related information increased prior to ED visits or hospitalizations, peaking a few days before the event.
Conclusions:
- Nursing notes are a valuable resource for early UTI risk identification in home care.
- Implementing NLP tools can enhance the extraction of clinical data from nursing notes for improved decision-making.
- Utilizing NLP for UTI detection can lead to better patient outcomes, quality improvement, and cost reduction.
