Background:

  • Woven EndoBridge (WEB) device shape modification (WShM) during follow-up is linked to poor angiographic outcomes.
  • Predisposing factors for WShM remain undetermined.
  • Rotational cone beam computed tomography (VasoCT) enables quantitative analysis of WEB morphology over time.

Purpose of the Study:

  • To quantitatively analyze WEB shape changes over time using VasoCT.
  • To identify factors influencing WEB shape modification (WShM).
  • To explore strategies for reducing WShM and improving angiographic results.

Main Methods:

  • Retrospective analysis of patients treated with WEB devices (October 2015 - January 2019).
  • Systematic VasoCT acquisitions post-implantation and during follow-up.
  • Quantification of WShM as percentage reduction in distance between WEB markers.

Main Results:

  • Mean WShM was 48%±24 at a mean follow-up of 15.5 months.
  • Significantly higher WShM in aneurysm recurrence group compared to adequate occlusion group (51% vs 36%).
  • WEB width oversizing significantly correlated with WShM reduction (r=-0.38, p=0.002).

Conclusions:

  • Woven EndoBridge (WEB) shape modification (WShM) can be mitigated through strategic width oversizing.
  • Appropriate WEB width selection may lead to improved angiographic outcomes.
  • Absence of WShM guaranteed complete aneurysm occlusion in this cohort.
