Summary
Delayed 18F-FDG PET/CT imaging can help differentiate hepatic lesions. In this case, a delayed scan revealed a benign cavernous hemangioma initially suspected as metastasis from invasive ductal carcinoma.

- A 72-year-old woman with invasive ductal carcinoma underwent neoadjuvant chemotherapy, quadrantectomy, and adjuvant radiotherapy.
- Two years post-treatment, a hepatic nodule was detected on CT, raising suspicion for metastasis.
Observation:
- Standard whole-body 18F-FDG PET/CT showed no abnormal uptake in the hepatic nodule.
- A delayed upper abdomen 18F-FDG PET/CT acquisition at 180 minutes demonstrated increased radiotracer uptake in the nodule.
Findings:
- Histological examination of the resected nodule confirmed it to be a benign cavernous hemangioma, not a metastasis.
- The delayed imaging protocol was crucial in identifying the true nature of the hepatic lesion.
Implications:
- Delayed 18F-FDG PET/CT may improve the diagnostic accuracy for hepatic lesions in cancer survivors.
- This technique could potentially reduce unnecessary invasive procedures and patient anxiety.
- Further research into delayed imaging protocols for indeterminate lesions is warranted.
