Background:

  • Teprotumumab is a monoclonal antibody targeting the insulin-like growth factor 1 (IGF-1) receptor.
  • IGF-1 receptors are implicated in Graves ophthalmopathy and are also present in the cochlea.
  • Previous clinical trials noted self-limited audiologic symptoms in 5 participants, but objective data are lacking.

Observation:

  • A 61-year-old female with Graves ophthalmopathy developed bilateral hearing loss, sound distortion, and tinnitus after teprotumumab treatment.
  • Audiometry revealed mild sloping to moderately-severe sensorineural hearing loss.
  • Hearing loss remained unchanged 4 months after discontinuing teprotumumab and initiating corticosteroid treatment.

Findings:

  • This report describes one of the first cases of teprotumumab-induced irreversible sensorineural hearing loss.
  • The observed ototoxicity resulted in persistent hearing impairment despite treatment cessation and intervention.

Implications:

  • Teprotumumab-induced ototoxicity can lead to irreversible sensorineural hearing loss.
  • Periodic audiologic evaluations are recommended for patients undergoing teprotumumab therapy.
  • Further research is needed to understand the mechanisms and prevalence of teprotumumab-related hearing loss.
