Background:

  • Merkel cell carcinoma (MCC) is a rare primary neuroendocrine neoplasm of the skin.
  • It typically affects elderly individuals with a history of sun exposure.
  • MCC usually presents as a dermal tumor, with intraepidermal involvement being uncommon.

Observation:

  • A 90-year-old male presented with an erythematous, desquamative plaque on his left cheek.
  • Histopathology revealed an intraepidermal lesion with multifocal epithelial growth.
  • Microscopic examination showed small cells with scant cytoplasm, hyperchromatic nuclei, molding, and high mitotic/apoptotic rates.

Findings:

  • Immunohistochemistry was positive for CK20, CK7, synaptophysin, and INSM1.
  • These findings confirmed a diagnosis of Merkel cell carcinoma in situ.
  • The intraepidermal presentation alone is exceptionally rare for this neoplasm.

Implications:

  • This case underscores the importance of considering Merkel cell carcinoma in situ in the differential diagnosis of intraepidermal neoplasms.
  • Accurate diagnosis is crucial, necessitating the exclusion of other neoplasms with potential intraepidermal forms.
  • The clinical evolution and prognosis of intraepidermal Merkel cell carcinoma remain largely unknown.
