Background:

Wilms' tumor, also known as nephroblastoma, is an exceptionally rare kidney malignancy in adults.
This report details a case of adult Wilms' tumor presenting as a large renal mass.

Observation:

A 39-year-old female presented with right flank pain.
Imaging revealed a substantial (128x100 mm) mass in the upper right kidney.

Findings:

Morphological and immunohistochemical analysis confirmed the diagnosis of adult Wilms' tumor post-nephrectomy.
Histopathologic exclusion of renal cell carcinoma was key to the diagnosis.

Implications:

The case underscores the importance of considering adult Wilms' tumor in patients with flank pain and renal masses.
