Idiopathic lenticular surface neovascularization: An unusual presentation

This case report details the first instance of isolated idiopathic posterior lenticular surface neovascularization in a healthy patient. The condition, affecting the lens capsule, shows no signs of progression with conservative management.

- AS-OCT demonstrated a unique finding of a hyperreflective layer on the posterior lens capsule.
- Standard ocular imaging (B-scan, UBM) and systemic evaluation were unremarkable.
- No progression of neovascularization was observed over a six-month follow-up with conservative management.
Implications:
- This is the first reported case of isolated idiopathic posterior lenticular surface neovascularization.
- Potential triggers include posterior capsule breach or undetermined cyclitis, requiring further histopathological study.
- Asymptomatic cases can be managed conservatively with close monitoring.
