Summary
Spontaneous intercostal lung hernia, a rare condition, can occur after heavy lifting. Conventional surgical repair offers low morbidity and good long-term results for this condition.

- A 46-year-old male with hypertension, heart failure, and diabetes presented with spontaneous intercostal lung hernia after heavy lifting.
- The hernia was complicated by hematoma, rib fracture, hemothorax, diaphragmatic laceration, and omentum herniation.
- Imaging confirmed a stable intercostal lung hernia.
Findings:
- Surgical repair was performed using a conventional thoracotomy approach.
- The patient experienced low morbidity and a good postoperative outcome.
- No recurrence or thoracic deformity was noted at 2-year follow-up.
Implications:
- This case highlights the successful management of a complex spontaneous intercostal lung hernia.
- Conventional surgical techniques remain effective for intercostal lung hernias.
