Summary

A rare paratesticular pleomorphic sarcoma caused scrotal swelling in a young male. Multi-modality imaging aided diagnosis and surgical planning for this extratesticular tumor.

  • Paratesticular tumors are rare but significant causes of scrotal swelling.
  • Early and accurate diagnosis is crucial for effective management.

Observation:

  • A 23-year-old male presented with left hemiscrotal swelling.
  • Imaging revealed a heterogeneous, predominantly solid paratesticular mass with internal vascularity and cystic areas.
  • MRI showed hemorrhage and fat plane loss with the corpora cavernosa.

Findings:

  • Histopathology confirmed an undifferentiated high-grade pleomorphic sarcoma.
  • Preoperative imaging (ultrasound, CT, MRI) was essential for surgical planning.
  • No locoregional or distant metastases were identified on CT.
