    Summary

    Tricalcium phosphate (TCP) demonstrated effective bone regeneration in large orthopedic defects in pigs. TCP showed comparable or superior bone ingrowth compared to autogenous bone grafts.

    Background:

    • Tricalcium phosphate (TCP) is a resorbable ceramic scaffold for bone ingrowth.
    • TCP shows promise in small periodontal defects but its efficacy in larger orthopedic defects is less understood.

    Purpose of the Study:

    • To evaluate the bone ingrowth potential of tricalcium phosphate in large cancellous bone defects (up to 12 cm3) in adult pigs.
    • To compare TCP's performance against autogenous bone grafts in these defects.

    Main Methods:

    • Skeletally mature pigs underwent large metaphyseal defects in the tibia and femur.
    • Defects were filled with either TCP or autogenous bone graft (control).
    • Bone ingrowth was assessed histomorphometrically at nine months.

    Main Results:

    • No inflammation was observed; TCP was surrounded by viable bone and largely resorbed by nine months.
    • Tibial defects showed significantly higher bone replacement with TCP (32%) versus controls (13%).
    • Femoral defects filled with TCP demonstrated comparable bone regeneration to autogenous bone (29%).

    Conclusions:

    • Tricalcium phosphate is a viable bone graft substitute for large orthopedic defects in pigs.
    • TCP facilitates significant bone ingrowth and replacement, comparable to autogenous bone in certain defect sizes.
