This case study shows nivolumab effectively treated advanced renal cell carcinoma, making previously unresectable tumors operable. The therapy led to significant tumor reduction and improved patient outcomes.
Area of Science:
- Oncology
- Immunotherapy
- Nephrology
Background:
- Advanced renal cell carcinoma (RCC) presents significant treatment challenges.
- Metastatic RCC often requires systemic therapy for unresectable disease.
Observation:
- A 65-year-old male with stage IV RCC (cT4N1M1b) showed progression on pazopanib.
- Initial nivolumab treatment caused temporary mobility issues due to vertebral metastases.
- Disease stabilization allowed resumption of nivolumab therapy.
Findings:
- Nivolumab treatment resulted in a 70% reduction of the primary renal tumor.
- Significant tumor regression or disappearance was observed in lung and bone metastases.
- Neoadjuvant nivolumab therapy rendered the previously unresectable tumor amenable to radical nephrectomy.
Implications:
- Nivolumab demonstrates efficacy as a neoadjuvant therapy for advanced, unresectable renal cell carcinoma.
- This case highlights the potential for immunotherapy to convert unresectable tumors to a resectable state.
- Successful conversion to surgical resection after immunotherapy offers a new treatment paradigm for advanced RCC.
