Posterior basal (S10) segmentectomy is challenging. This study presents two techniques, including understanding vessel/bronchi locations and using suction-guided stapling, to improve uniportal operations for this uncommon pulmonary procedure.
Area of Science:
- Thoracic Surgery
- Minimally Invasive Surgery
- Pulmonary Surgery
Background:
- Posterior basal (S10) segmentectomy is a complex and infrequently performed pulmonary procedure.
- Uniportal approaches present unique challenges for thoracic surgeons, especially in S10 segmentectomies.
Observation:
- Preoperative three-dimensional computed tomography/broncho-angiography is crucial for understanding the anatomical relationships of pulmonary vessels and bronchi.
- The "suction-guided stapling" technique facilitates dissection and division of peripheral pulmonary vessels and bronchi during uniportal surgery.
Findings:
- Successful application of two key techniques enabled a challenging uniportal posterior basal (S10) segmentectomy.
- Detailed preoperative imaging and a novel stapling method were instrumental in the procedure's success.
Implications:
- These techniques can enhance the feasibility and safety of uniportal S10 segmentectomies.
- Improved understanding and technical advancements may lead to wider adoption of minimally invasive approaches for complex pulmonary resections.
