Cardiac surgery in nonagenarians showed zero 6-month mortality in a contemporary cohort. These findings support offering cardiac surgery to carefully selected nonagenarian patients.
Area of Science:
- Cardiology
- Geriatric Medicine
- Surgical Outcomes
Background:
- Limited data exists on cardiac surgery outcomes in nonagenarians.
- This population is increasingly referred for cardiac interventions.
- Contemporary peri-operative care advancements necessitate outcome evaluation.
Purpose of the Study:
- To describe patient characteristics, management, and outcomes of nonagenarians undergoing cardiac surgery.
- To assess 6-month mortality in this cohort.
- To detail postoperative complications in nonagenarians.
Main Methods:
- Retrospective observational study of 18 nonagenarians undergoing cardiac surgery via median sternotomy.
- Exclusion of transcatheter aortic valve implantation (TAVI) and thoracic endovascular aortic repair (TEVAR).
- Data collected from four Australian hospitals over 8 years (2012-2019).
Main Results:
- Median age was 91 years; comorbidities and frailty were common.
- Zero intra-operative, in-hospital, or 6-month mortality observed.
- 16.7% experienced Grade IIIb complications; 16.7% had 30-day readmissions.
Conclusions:
- Zero 6-month postoperative mortality in a selected contemporary cohort of nonagenarians.
- Findings support offering cardiac surgery to carefully selected nonagenarian patients.
- Further research may explore long-term outcomes and specific surgical indications.
