General Anesthesia: Overview

273
Anesthesia is a medical procedure that uses drugs for CNS suppression to enable painless surgeries and procedures. The selection of anesthetics is influenced by their pharmacokinetic properties, side effects, and patient characteristics. Various types of anesthesia include general, local, regional, spinal, and inhalational.
General anesthesia induces unconsciousness in the whole body, while the others target specific areas or sensations. It is administered to minimize adverse effects, maintain...

Parenteral Anesthetics: Overview

188
Intravenous anesthetics are drugs administered parenterally to induce anesthesia or sedation. Propofol is a widely used agent formulated as a 1% emulsion in soybean oil, glycerol, and egg phosphatide. It induces rapid anesthesia primarily due to its rapid distribution from the bloodstream to target tissues and is metabolized in the liver. However, it can cause significant pain on injection and hypertriglyceridemia. Fospropofol, a water-based prodrug of propofol, lacks these adverse effects.

Area of Science:

  • Cardiovascular medicine
  • Medical device technology
  • Anesthesiology

Background:

  • The US FDA approved the Aveir VR Leadless Pacemaker, a novel nontransvenous cardiac pacing system.
  • This device joins the Micra transcatheter pacing system, expanding options for leadless cardiac pacing.
  • The Aveir VR features design enhancements over its predecessor, the Nanostim.

Purpose of the Study:

  • To highlight the unique characteristics of the Aveir VR Leadless Pacemaker.
  • To inform anesthesiologists about considerations for managing patients with the Aveir VR.
  • To differentiate the Aveir VR from other leadless pacing systems like the Micra.

Main Methods:

  • Review of the Aveir VR Leadless Pacemaker's design and features.
  • Comparison with existing leadless pacing technologies (Micra, Nanostim).
  • Analysis of potential perioperative implications for anesthesiology.

Main Results:

  • The Aveir VR possesses distinct design elements compared to the Micra.
  • These differences necessitate specific preoperative and perioperative management strategies.
  • Existing guidelines for transvenous pacemakers may not fully apply.

Conclusions:

  • Anesthesiologists must be aware of the Aveir VR's unique features for safe patient care.
  • Understanding the differences between leadless pacemakers is essential for perioperative planning.
  • Further research may be needed to establish specific anesthetic protocols for the Aveir VR.
