Purpose of the Study:

  • To evaluate the clinical efficacy of staged surgery for complex closed Pilon fractures.

  • A prospective study of 29 patients with complex closed Pilon fractures treated with staged surgery.
  • Fracture classification (Ruedi-Allgower), surgical timing, healing duration, and complications were recorded.
  • Clinical outcomes were assessed using the American Orthopedic Foot and Ankle Society (AOFAS) score, and radiological reduction using the Burwell-Charnley system.

Main Results:

  • All 29 fractures healed within 2-6 months with no major complications like infection or stiffness.
  • Significant improvement in AOFAS scores from 34.11 pre-operatively to 90.10 post-operatively (P<0.05).
  • Satisfactory to excellent radiological reduction was achieved in 27 out of 29 patients.

  • Staged surgery provides a safe and effective treatment for complex closed Pilon fractures.
  • This method results in fewer complications, good fracture reduction, and stable fixation.
  • Staged surgery facilitates good recovery of ankle joint function.
