Manager and staff perceptions of nursing leadership and climate differ, especially with more experienced staff. These differences in implementation leadership perception correlate with poorer staff-rated implementation climate.
Area of Science:
- Nursing Management
- Organizational Psychology
- Healthcare Implementation Science
Background:
- Literature on staff-manager perceptual differences in nursing social dynamics is limited.
- Understanding these differences is crucial for effective implementation strategies and improved patient/organizational outcomes.
Purpose of the Study:
- To analyze perceptual differences in implementation leadership and climate between nursing staff and managers.
- To investigate factors associated with these perceptual discrepancies.
Main Methods:
- Secondary analysis of cross-sectional survey data (2016-2017) from 22 nursing units in 7 US hospitals.
- Utilized structural equation modeling (SEM) with latent difference score (LDS) models to analyze data from 261 registered nurses and 22 nurse managers.
- Assessed associations between LDSs and manager scores, nursing experience, and unit tenure.
Main Results:
- Higher manager ratings on Implementation Leadership Scale (ILS) and Implementation Climate Scale (ICS) correlated with larger perceptual differences.
- Increased years of nursing experience were linked to greater ILS and ICS perceptual differences.
- Greater perceptual differences in implementation leadership were associated with lower staff-reported implementation climate.
Conclusions:
- Significant relationships exist between manager scores, staff-manager perceptual differences, and staff-reported climate in nursing.
- The reasons for and solutions to these perceptual differences in implementation leadership and climate remain unclear.
- Further research is needed to explore the impact of perceptual differences on implementation and patient outcomes.
