Sleep disturbances, particularly periodic limb movements of sleep (PLMS) and severe oxygen desaturations, are linked to increased cancer risk. Cluster analysis identified distinct sleep phenotypes associated with malignancy.
Area of Science:
- Sleep Medicine
- Oncology
- Biostatistics
Background:
- Cellular processes are regulated by sleep; disruptions may increase cancer risk.
- Polysomnography (PSG) measures sleep disturbances.
- Investigating the link between PSG phenotypes and cancer is crucial.
Purpose of the Study:
- To determine the association between PSG-defined sleep disturbances and incident cancer.
- To validate cluster analysis for identifying PSG phenotypes.
Main Methods:
- Retrospective multicenter cohort study (1994-2017) of adults without cancer at baseline.
- Used linked clinical and administrative data from Ontario, Canada.
- Identified PSG phenotypes using k-means cluster analysis and Cox regressions.
Main Results:
- 29,907 individuals studied; 8.4% developed cancer.
- Five PSG clusters identified: mild, poor sleep, severe OSA/fragmentation, severe desaturations, and PLMS.
- PLMS and severe desaturations showed a significant association with incident cancer after controlling for confounders.
Conclusions:
- PSG phenotypes are important in cancer risk assessment.
- PLMS and oxygen desaturation may play a role in cancer development.
- A tool was developed to classify patients into identified PSG clusters.
