    Summary

    Mental calculation can trigger epileptic seizures or epileptiform discharges, a rare phenomenon. This case study highlights a patient whose electroencephalogram showed epileptiform activity during difficult mental arithmetic tasks.

    Background:

    • A 34-year-old accountant with a 20-year history of generalized tonoclonic seizures was studied.
    • Clinical seizures were not associated with mental arithmetic performance.

    Findings:

    • Electroencephalographic (EEG) epileptiform discharges were observed during challenging mental calculation.
    • These discharges did not significantly affect the patient's accuracy in solving mathematical problems on most occasions.

    Implications:

    • This case expands the understanding of reflex epilepsy triggers.
    • It suggests a potential link between complex cognitive tasks and epileptiform activity in susceptible individuals.
    • Further research may explore the neural mechanisms underlying cognitive-exacerbated epilepsy.
