Purpose of the Study:

  • To establish trimester-specific thyroid function test reference values for Sri Lankan pregnant women.
  • To compare these values with regional and international data.
  • To determine the prevalence of Thyroid Peroxidase (TPO) antibody positivity and iodine deficiency.

Main Methods:

  • A cross-sectional study excluded participants with thyroid disease, abnormal thyroid ultrasounds, or high-risk factors.
  • An "ideal-reference population" was established across trimesters.
  • Thyroid hormones were measured using chemiluminescence; reference ranges were derived using median and centiles.

Main Results:

  • The final sample comprised 369 women.
  • Trimester-specific TSH reference ranges were established: 0.014-2.77 mIU/L (1st), 0.31-3.2 mIU/L (2nd), and 0.34-3.4 mIU/L (3rd).
  • A weak correlation was found between TPO antibody levels and TSH; no association was observed between urine iodine and thyroid function.

Conclusions:

  • The derived TSH reference ranges are more concordant with Caucasian values than regional ones.
  • Methodological differences in study design, reference population selection, and hormone measurement assays may explain observed discrepancies.
