Background:

Remote patient monitoring (RPM) is expanding, but concerns exist about exacerbating health disparities.
Standardized care pathways aim for equitable patient management, including RPM referrals.

Purpose of the Study:

To investigate if enrollment in a COVID-19 RPM program was offered equitably across different demographic groups.
To assess the impact of social vulnerability and urbanicity on RPM referral rates.

Main Methods:

Retrospective cohort study of 16,739 COVID-19 patients from March-September 2020.
Used CDC Social Vulnerability Index (SVI) to quantify vulnerability.
Logistic regression analyzed odds ratios (ORs) for RPM referral based on SVI and urbanicity.

Main Results:

17.6% of patients were referred for RPM.
Higher social vulnerability was associated with lower referral odds (OR 0.73).
Urbanicity was a significant predictor, with metropolitan/urban patients most likely to be referred, contrary to initial hypotheses.

Conclusions:

Disparities in RPM access were observed despite standardized care pathways.
Health systems must critically evaluate RPM implementation to ensure equitable care delivery.
Addressing social determinants of health is crucial for equitable digital health adoption.
