Ultra performance liquid chromatography electrospray ionization triple quadrupole mass spectrometry can identify paraprotein subtypes. This mass spectrometry method shows high agreement with immunofixation electrophoresis, aiding in detecting drug interference and biclonal paraproteins.
Area of Science:
- Biochemistry
- Analytical Chemistry
- Immunology
Background:
- Mass spectrometry offers enhanced specificity and sensitivity for paraprotein identification.
- Ultra performance liquid chromatography (UPLC) coupled with electrospray ionization triple quadrupole mass spectrometry (ESI-MS/MS) is a powerful tool for qualitative analysis.
Purpose of the Study:
- To demonstrate the capability of UPLC-ESI-MS/MS for qualitative paraprotein analysis.
- To evaluate the method's performance against established techniques like immunofixation electrophoresis (IFE).
Main Methods:
- Immunopurification of paraproteins from patient serum (n=40) using camelid antibodies.
- Reduction of immunoglobulins to separate heavy and light chains.
- UPLC-ESI-MS/MS analysis of reduced samples, followed by mass spectral peak deconvolution to determine light chain mass.
Main Results:
- A 92.5% agreement was observed between chromatographic patterns and mass information.
- Mass spectrometry data showed a 90% correlation with IFE results.
- Identified high mass light chains (>25 kDa) suggestive of glycosylation and confirmed Daratumumab interference.
Conclusions:
- UPLC-ESI-MS/MS is feasible for identifying paraprotein subtypes.
- The method can screen for therapeutic antibody (tAb) interference and identify biclonals of the same isotype.
