Subcutaneous leiomyosarcomas (LMS), a rare soft tissue sarcoma, can present in the anterior abdominal wall. This case highlights successful surgical management and adjuvant radiation for a large supra-pubic LMS.

Background:

  • Subcutaneous leiomyosarcomas (LMS) are rare soft tissue sarcomas originating from small-to-medium-sized blood vessels.
  • Anterior abdominal wall involvement by LMS is exceptionally uncommon.

Observation:

  • A 72-year-old male presented with a 3-month history of a rapidly growing supra-pubic mass.
  • The mass extended to the penile root, with magnetic resonance imaging (MRI) revealing a large, heterogeneous lower abdominal mass indenting the penile corpora.
  • No regional lymphadenopathy was detected.

Findings:

  • Complete pathological clearance was achieved through wide excision, including a total penectomy and cutaneous urethrostomy.
  • The patient received adjuvant radiation therapy post-surgery.

Implications:

  • This case demonstrates a rare presentation of subcutaneous leiomyosarcoma involving the anterior abdominal wall and penis.
  • Successful management involved aggressive surgical resection and adjuvant radiotherapy, leading to a favorable outcome with no recurrence at two years.
  • Highlights the importance of considering rare soft tissue sarcomas in the differential diagnosis of abdominal masses.
