Background:

  • Community engagement is recognized for its potential to address vaccination hesitancy and build confidence.
  • Despite widespread support for community engagement in health, evidence for its impact on vaccination promotion is fragmented and of variable quality.

Purpose of the Study:

  • To systematically review the effectiveness of various community engagement strategies in promoting vaccination rates.
  • To analyze the impact of different contents and extents of community engagement on vaccination uptake.

Main Methods:

  • A systematic literature search was conducted across multiple databases (PubMed, Embase, Web of Science, Cochrane Library, CNKI, Wan Fang) following PRISMA guidelines.
  • Included original research articles with experimental designs evaluating community engagement for vaccination promotion.
  • Data extraction and quality assessment were performed by two independent reviewers, with discrepancies resolved by a third reviewer.

Main Results:

  • A meta-analysis of 20 articles (2006-2021) showed significant increases in vaccination rates.
  • Participant recruitment (RD 0.51) and high community engagement extent (RD 0.49) yielded the largest effect sizes.
  • Health service support and health education/discussion were the most effective engagement contents.

Conclusions:

  • Community engagement is an effective strategy for vaccination promotion, with effectiveness varying by content and extent.
  • A "fit-for-purpose" approach, rather than a "one-size-fits-all" strategy, is recommended to maximize the impact of community engagement on vaccination rates.
