A 78-year-old man with recurrent prostate cancer showed unexpected findings on a Gallium-68 prostate-specific membrane antigen-11 (68Ga-PSMA-11) PET/CT scan. A mildly avid soft tissue lesion was a rare sarcoma, not prostate cancer, highlighting a potential imaging pitfall.
