Summary

COVID-19 vaccination significantly reduced severe illness and hospitalizations in healthcare workers (HCWs). However, breakthrough infections were more common in HCWs with direct patient contact and lower antibody levels.

Background:

Purpose of the Study:

  • To assess SARS-CoV-2 infection and breakthrough infection rates in fully vaccinated HCWs.
  • To investigate the correlation between neutralizing antibody response and breakthrough infections.

Main Methods:

  • Retrospective cohort study involving 1079 HCWs at Dhulikhel Hospital from December 2021 to October 2022.
  • Semi-structured interviews (in-person/phone) and a prospective serological cohort of 137 HCWs.
  • Analysis of infection rates, severity, hospitalization needs, and antibody levels in relation to vaccination status and patient contact.

Main Results:

  • Over half (54.7%) of participants had a prior SARS-CoV-2 infection.
  • 35% experienced breakthrough infections post-full vaccination.
  • Infections after vaccination resulted in less severe illness and fewer hospitalizations compared to pre-vaccination infections.
  • HCWs with direct patient care roles had a higher risk of breakthrough infection.
  • Lower antibody levels correlated with a higher incidence of breakthrough infections (35.3%) compared to moderate/high levels (11.9%).

Conclusions:

  • COVID-19 vaccination effectively reduces disease severity and hospitalization needs.
  • Breakthrough infections are more prevalent among HCWs in direct patient care roles.
  • Maintaining adequate antibody levels is associated with a lower risk of breakthrough infections.
