Background:

  • Bladder exstrophy (BE) often results in decreased penile length due to pubic symphysis diastasis and corporal body separation.
  • Intrinsic penile shortening, dorsal curvature, and scarring further contribute to reduced penile length in BE patients.

Purpose of the Study:

  • To evaluate the necessity and benefits of penile reconstruction in adult men previously treated for bladder exstrophy (BE).
  • To assess outcomes of penile lengthening and simultaneous repair of associated penile pathologies in BE patients.

Main Methods:

  • Retrospective review of 31 adult male patients with repaired BE (mean age 21.4 years).
  • Surgical techniques included sub-periosteal detachment of corporal bodies, dermal grafting for penile resurfacing, urethroplasty, and phalloplasty.
  • Patient-reported outcomes and objective measurements of penile length increase were assessed.

Main Results:

  • 80.6% of patients reported satisfactory outcomes following penile reconstruction.
  • Penile lengthening ranged from 50% to 150% of preoperative length, measured at 6 months and 1 year postoperatively.
  • Successful correction of short phallus, dorsal chordee, and distal urethral anomalies was achieved.
