Background:

  • Osteoradionecrosis of the jaw (ORN) is a severe complication following radiation therapy for head and neck cancer (HNC).
  • Accurate identification of risk factors and severity is crucial for patient management and treatment planning.

Purpose of the Study:

  • To identify risk factors associated with the development of ORN in HNC patients treated with intensity-modulated radiation therapy (IMRT).
  • To develop and validate a novel classification system for ORN severity.

Main Methods:

  • A cohort of 2,732 HNC patients treated with IMRT (≥45 Gy) between 2011-2017 were analyzed.
  • Multivariable logistic regression identified ORN risk factors and stratified patients into high- and low-risk groups.
  • A novel classification system, ClinRad, was developed based on bone necrosis extent and clinical signs, and compared against 15 existing systems.

Main Results:

  • ORN occurred in 8% of patients; high-risk factors included oral/oropharyngeal primaries, IMRT dose ≥60 Gy, smoking, and advanced periodontal disease.
  • High-risk patients had a significantly higher ORN rate (12.7%) vs. low-risk (3.1%).
  • The novel ClinRad system outperformed existing classifications in identifying serious ORN events (jaw fracture, resection) with 5.7% detection rate.

Conclusions:

  • Key risk factors for ORN in HNC patients treated with IMRT were identified.
  • The novel ClinRad classification system effectively depicts ORN severity and outperforms existing systems.
  • ClinRad may improve clinical care and facilitate future clinical trials for ORN management.
