Cancer Survival Analysis01:21

Cancer Survival Analysis

863
Cancer survival analysis focuses on quantifying and interpreting the time from a key starting point, such as diagnosis or the initiation of treatment, to a specific endpoint, such as remission or death. This analysis provides critical insights into treatment effectiveness and factors that influence patient outcomes, helping to shape clinical decisions and guide prognostic evaluations. A cornerstone of oncology research, survival analysis tackles the challenges of skewed, non-normally...

Area of Science:

  • Radiation Oncology
  • Medical Physics
  • Cancer Treatment Planning

Background:

  • Volumetric Modulated Arc Therapy (VMAT) is a sophisticated radiation technique for prostate cancer treatment.
  • Developing generalized models for VMAT plan quality is challenging due to institutional variations in treatment protocols.
  • Knowledge-based planning (KBP) models aim to automate and optimize radiation therapy planning.

Purpose of the Study:

  • To evaluate the clinical effectiveness of a broad, multi-institutional KBP model (KBPbroad) for prostate cancer VMAT plans.
  • To compare the quality of plans generated by KBPbroad against institutional-specific KBP models (KBPonsite) and clinical plans (CPs).
  • To assess KBPbroad's performance across different institutions with varying treatment protocols.

Main Methods:

  • A KBPbroad model was trained on 561 prostate cancer VMAT plans from five institutions.
  • An open-loop evaluation was conducted at two independent institutions using 30 patient cases each.
  • Plans generated by KBPbroad were compared to CPs and KBPonsite plans based on target coverage and organ-at-risk (OAR) dose constraints.

Main Results:

  • KBPbroad maintained target coverage comparable to CPs while meeting dose constraints.
  • At institution X, KBPbroad significantly reduced rectal wall dose volumes (V40Gy, V60Gy, V70Gy) compared to KBPonsite and CPs.
  • At institution Y, KBPbroad demonstrated superior dose homogeneity and conformality for the planning target volume and improved OAR sparing compared to CPs.

Conclusions:

  • The broad KBP model (KBPbroad) is clinically effective and can improve VMAT plan quality, particularly at institutions with less optimized OAR dose reduction.
  • KBPbroad offers a viable solution for enhancing VMAT planning consistency and quality across diverse clinical settings.
  • The study validates the potential of generalized KBP models to outperform single-institution models and standard clinical practices.
