Context:

Cardiovascular diseases are a leading cause of death and economic burden in Chile.
Existing healthcare systems in Chile lack a fully integrated approach to managing cardiometabolic conditions.
The FUSAT Clinical Hospital sought to address these challenges through innovative care delivery.

Purpose:

To describe the structured process of establishing a Cardiometabolic Integrated Practice Unit (UPI).
To adapt a recognized framework (Porter and Lee) for implementing the UPI.
To outline the key steps involved in organizing the UPI for optimal patient care.

Summary:

The Cardiometabolic UPI was structured using five defined steps: patient selection, needs-based stratification, care delivery, communication strategy, and monitoring indicators.
Implementation involved a multidisciplinary team working collaboratively over six months.
The UPI integrates care across the entire patient journey, from promotion and prevention to rehabilitation.

Impact:

Successful implementation of the Cardiometabolic UPI at the FUSAT Clinical Hospital.
Demonstrates the feasibility of such integrated projects through defined governance and strategic objectives.
Highlights the potential for significant positive impact on cardiovascular healthcare delivery in Chile.
