    Central nervous system (CNS) involvement is a rare complication of disseminated Kaposi's sarcoma. These two cases highlight the importance of considering CNS tumors in Kaposi's sarcoma patients with neurological symptoms.

    Background:

    • Kaposi's sarcoma (KS) is an opportunistic neoplasm commonly associated with human herpesvirus 8 (HHV-8).
    • Disseminated KS typically affects the skin, lymph nodes, and visceral organs.
    • Central nervous system (CNS) involvement is an uncommon but serious manifestation of disseminated KS.

    Observation:

    • Two male patients with disseminated Kaposi's sarcoma developed metastatic lesions in the CNS.
    • One patient was homosexual and had a known risk factor for KS.
    • The second patient had no identifiable risk factors for disseminated KS, presenting a diagnostic challenge.

    Findings:

    • The cases demonstrate that CNS metastasis can occur in disseminated Kaposi's sarcoma.
    • Neurologic signs and symptoms in KS patients may indicate CNS involvement.
    • Early suspicion and diagnosis of CNS lesions are crucial for appropriate management.

    Implications:

    • Clinicians should maintain a high index of suspicion for CNS involvement in patients with Kaposi's sarcoma presenting with neurological deficits.
    • Diagnostic workup for CNS lesions should be considered in this patient population.
    • Further research is warranted to understand the pathogenesis and optimize treatment strategies for CNS Kaposi's sarcoma.
