Purpose of the Study:

  • To evaluate a 16-year experience with recto-urinary fistulas (RUFs).
  • To understand the consequences and management of radiation-induced RUFs in prostate cancer patients.

Main Methods:

  • Retrospective review of electronic medical records from 2004-2020.
  • Analysis of patient demographics, clinical presentation, diagnostics, and surgical management.
  • Identification of seven patients with radiation-induced RUFs.

Main Results:

  • All seven patients were male, with an average age of 66 at diagnosis.
  • Radiation proctitis with rectal ulceration preceded fistula in 6 of 7 patients.
  • Common symptoms included fecaluria, pneumaturia, and rectal pain; CT was the primary diagnostic tool. Management involved diversion in all patients, with only one definitive repair.

Conclusions:

  • Recto-urinary fistulas are a dreaded, albeit rare, complication of prostate cancer radiation.
  • Radiation proctitis with rectal ulceration is a precursor and requires aggressive management.
  • While diversion manages symptoms, definitive repair is limited to select patients due to comorbidities or mortality.
