[A Study of the Invasiveness of Robotic Surgery with da Vinci Si or Xi for Rectal Cancer]

Background:
The da Vinci surgical system has been updated from Si to Xi with several new features. This study investigated the invasiveness of robot-assisted rectal cancer surgery performed at our hospital using Si and Xi.
Methods:
Of the 124 robot-assisted rectal cancer surgeries performed between August 2018 and January 2024, we compared 60 cases using Si(Si group)and 62 cases using Xi(Xi group), excluding 2 cases of total pelvic exenteration.
Results:
There was no difference in patient background and pathologic factors between the Si and Xi groups; however, tumor localization was lower in the Xi group. The Xi group had significantly lower operative time, blood loss, complications of Clavien-Dindo(CD)classification Grade ≥Ⅱ, and postoperative hospital stay. White blood cell count on postoperative day 3 and C-reactive protein (CRP)levels on postoperative days 1, 3, and 7 were significantly lower in the Xi group, as in the case of patients with CD complication Grade ≥Ⅱ were excluded from the study.
Conclusion:
The results suggest that the Xi system and development of a more standardized and proficient surgical technique may help achieve less invasive rectal cancer surgery.
