Summary

Lateral cephalometric radiographs may approximate facial profiles on average, but individual patient accuracy is often clinically insufficient. Caution is advised when using these 2D images to assess 3D facial soft-tissue profiles.

Area of Science:

  • Orthodontics
  • Medical Imaging
  • Anthropometry

Background:

  • Lateral cephalometric radiographs are commonly used to assess facial soft-tissue profiles.
  • The accuracy of these 2D representations compared to true 3D profiles is not fully established.

Purpose of the Study:

  • To investigate the accuracy of facial soft-tissue profile lines in lateral cephalometric radiographs.
  • To compare cephalometric profiles with true profiles derived from 3D photographs.

Main Methods:

  • Prospective methodological study involving 100 orthodontic patients.
  • True profiles obtained from 3D surface models; cephalometric profiles from radiographs.
  • Procrustes superimposition used to compare landmark configurations and calculate interlandmark distances.

Main Results:

  • Statistically significant differences (median 0.84 mm) exist between cephalometric and true profiles on average.
  • Individual patient analysis revealed significant deviations: 40% >1 mm, 10% >2 mm.
  • Deviations exceeded 1 mm at the soft-tissue nasion area, potentially due to the cephalostat.

Conclusions:

  • Lateral cephalograms may offer adequate average facial profile representation.
  • Clinically significant errors are frequent when evaluating individual patients.
  • Caution is recommended when using lateral cephalograms for facial soft-tissue profile evaluation.
