    Background:

    • Nonconvulsive status epilepticus (NCSE) presents diagnostic challenges.
    • Previous studies suggested potential abnormalities in sensory processing during NCSE.
    • Electrophysiological tests (EPs) like PVERs, BAERs, and SSERs are used to assess sensory pathways.

    Observation:

    • EPs were performed on two patients with NCSE exhibiting behavioral changes and diffuse electrographic abnormalities.
    • The specific EP modalities utilized were Pattern Visual Evoked Responses (PVERs), Brainstem Auditory Evoked Responses (BAERs), and Somatosensory Evoked Responses (SSERs).
    • These EP modalities are known for their high reproducibility in healthy individuals.

    Findings:

    • Contrary to previous research, the ictal EPs in these NCSE patients were normal.
    • This indicates that sensory processing pathways, including brainstem, visual, and somatosensory cortices, were unaffected during the seizure events.
    • The findings challenge the notion that generalized ictal activity invariably impairs sensory pathway function.

    Implications:

    • Preserved sensory processing during NCSE suggests that electrophysiological assessments may not always reflect seizure impact on these pathways.
    • These results could refine our understanding of NCSE pathophysiology and diagnostic approaches.
    • Further research is warranted to explore the dissociation between electrographic abnormalities and preserved sensory function in NCSE.
