Tofacitinib showed efficacy in ulcerative colitis, but not Crohn's disease (CD). A patient with stricturing CD, previously excluded from trials, responded positively to tofacitinib treatment.
Area of Science:
- Gastroenterology
- Immunology
- Pharmacology
Background:
- Tofacitinib is an approved treatment for ulcerative colitis.
- Its efficacy in Crohn's disease (CD) is not well-established.
- Patients with stricturing CD were excluded from initial tofacitinib trials.
Observation:
- A case of severe stricturing colonic Crohn's disease is presented.
- The patient had previously failed other therapies.
Findings:
- The patient experienced a significant clinical response to tofacitinib therapy.
- Tofacitinib demonstrated effectiveness in this case of stricturing CD.
Implications:
- This case suggests a potential role for tofacitinib in managing stricturing Crohn's disease.
- Further investigation into tofacitinib's efficacy for this CD subtype is warranted.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...
