    Summary

Combined trepanotrabeculectomy and cataract extraction is a successful treatment for severe glaucoma and advanced cataracts. This combined procedure showed a high success rate with fewer complications than other methods.

    Area of Science:

    • Ophthalmology
    • Surgical Procedures
    • Glaucoma and Cataract Research

    Background:

    • Severe open-angle glaucoma and advanced cataracts often require combined surgical intervention for optimal visual outcomes.
    • Traditional combined procedures can be associated with significant postoperative complications, necessitating safer alternatives.

    Observation:

    • The study evaluated the efficacy and safety of combined trepanotrabeculectomy (TTE) and intracapsular cataract extraction.
    • Surgical outcomes and complications were monitored in operated eyes over a follow-up period of one to 11 years.

    Findings:

    • The combined TTE and cataract extraction procedure demonstrated an 84% success rate in operated eyes.
    • Complications were infrequent, with a notably lower incidence of postoperative flat anterior chamber compared to other combined procedures.
    • Surgical scars remained well-covered in 77% of eyes, without extraocular filtration or avascular cystic blebs.

    Implications:

    • Combined TTE and cataract extraction offers a safe and effective surgical option for patients with coexisting severe glaucoma and cataracts.
    • The procedure's low complication rate and high success suggest it as a preferred approach over separate surgical interventions.
    • Performing TTE before cataract extraction, if done separately, is recommended to preserve trabeculectomy function.
