Mass Spectrometry: Carboxylic Acid, Ester, and Amide Fragmentation

The fragmentation patterns observed for compounds such as carboxylic acids, esters, and amides in the mass spectra include ⍺-cleavage and McLafferty rearrangement. Fragmentation by ⍺-cleavage preferentially occurs at the carbon-carbon bond at the ⍺-position next to the carboxylic group to generate a neutral radical and a cation. Long chain compounds with hydrogen at their γ-carbon undergo McLafferty rearrangement to give a radical cation and a neutral alkene.

    • Antifibrinolytic drugs, such as tranexamic acid (TXA), are explored for improving surgical outcomes.

    Purpose of the Study:

    • To meta-analyze the impact of tranexamic acid (TXA) on complications in breast and body contouring procedures.
    • To evaluate the efficacy of different TXA administration routes (IV, topical, concurrent) on specific complications.

    Main Methods:

    • A systematic literature search was conducted on PubMed for studies involving TXA in breast reconstruction and body contouring.
    • A meta-analysis was performed on complications including hematoma, seroma, infection, and wound healing.
    • Random effects modeling (Mantel-Haenszel) was used to calculate odds ratios and confidence intervals, with subgroup analyses for administration methods.
