Topical carmustine therapy for lymphomatoid papulosis.

    Summary

    Topical carmustine (a nitrosourea compound) effectively reduced lymphomatoid papulosis lesions in seven patients. While it suppressed disease activity, long-term remissions were not achieved without continuous treatment.

    Background:

    • Lymphomatoid papulosis is a rare T-cell lymphoproliferative disorder characterized by papular and plaque-like skin lesions.
    • Current treatment options for lymphomatoid papulosis can be limited, necessitating exploration of novel therapeutic approaches.

    Observation:

    • Seven patients diagnosed with lymphomatoid papulosis were treated with topical carmustine solutions.
    • Treatment involved daily application of carmustine in dilute alcohol to the entire skin surface for 4-17 weeks.

    Findings:

    • All patients showed a rapid decrease in the number and size of their skin lesions.
    • Maintenance therapy with localized carmustine application suppressed disease activity and halved the lesion life cycle.
    • Bone marrow depression was not observed during the treatment period.

    Implications:

    • Topical carmustine demonstrates potential as an effective treatment for managing lymphomatoid papulosis symptoms.
    • Further research is warranted to optimize treatment protocols for achieving sustained remissions.
    • The absence of bone marrow toxicity suggests a favorable safety profile for topical carmustine in this context.
